Honoring Choices, Indiana - North Central is a not-for-profit organization whose mission is to proactively engage the people of our community in conversations with loved ones and medical caregivers about their goals for quality of life and Advance Care Planning. Who would speak for you if you could not speak for yourself in a health care crisis? Certified facilitators are available at no charge to educate and facilitate discussion and guidance to individuals and families who wish to develop an advance care plan, complete advance directives and designate a health care representative.
